When a couple with three children signed into their state insurance marketplace to renew coverage, they found that the cost had grown to $4,000 a month from $1,200 the year before. Other residents of Charlottesville, VA, which has just one health insurer, faced the same quandary because of lack of competition and changes imposed by Congress.
