As the open enrollment period for health insurance from the nation’s government-run marketplaces opened, Americans found a changed landscape: higher premiums and co-pays and, often, lower subsidies. With budgets slashed for advertising and navigators, which help people figure out which plan to select, consumers faced a bewildering array of choices.
